Investigation Summary
On June 13, 2002, Employee #1 was backing a manual pallet jack of materials into an aisle. A coworker was moving a load (consisting of 48 sheets of half-inch particle board weighing approximately 3900 pounds) with a powered industrial truck towards the dock area. The coworker had the load elevated during travel to clear material that had been placed in the aisle. When the coworker noticed Employee #1, he honked the horn and hit the brakes on the lift. The load of particle board shifted, the bands securing the load failed, all 48 sheets of particle board slipped off the forks, struck Employee #1, and killed him. Interviews indicated that there was a reoccurring problem with the aisle being blocked, causing the powered industrial truck operators to elevate loads as they were moving.
