Employee trips and falls off roof

Employee #1 and two coworkers had arrived at a roofing site at approximately 10:00 a.m. One coworker took three rolls of tar paper up to the roof in preparation for the roofing job. Employee #1 also went onto the roof which was a 6-in. 12-pitch roof measuring approximately 20 ft from ground to eave. Employee #1 was pulling an air hose and laying it out on the roof for his nail gun. As Employee #1 was laying the hose out on the roof, he stepped on the hose. This caused Employee #1 to lose his balance and fall down the pitch of the roof and onto a concrete patio. None of the employees were wearing personal fall protection, nor had they placed any other type of fall protection in place such as slide guards or a catch platform or scaffold. Employee #1 later died as a result of his injuries.
