Investigation Summary
On July 7, 2001, Employee #1 was preparing for moving a drilling rig the following morning. Three employees used an on-site derrick truck to move two pipe racks away from the rig and across the well site. Employee #1, the newest, went into and between the racks to move them apart after they were dragged a distance. The racks shifted suddenly and struck the employee's head, resulting in death. Subsequent investigation found the vehicle to be in poor working order and the rigging training program to be highly inadequate. Bilingual language difficulties existed on site with training and warning postings.
