Employee Killed After Fallling From Ladder To Reach A Mast

At 3:30 p.m. on January 10, 2001, Employee #1, an employee for International Shipbreaking Limited L.L.C., died when he fell while climbing an exterior ladder to reach the top of the H-Frame mast to secure the mast to the crane's sling. Employee #1 was climbing an exterior permanent ladder 42 feet long to reach the top of the H-Frame mast. The task was for two employees to get on top of the H-Frame mast to secure it to the crane's sling. The ladder was in six sections 7 feet long. It appears the four bolts attached to the mast had corroded from the inside. A section of the ladder came off from the four bolts and nuts anchoring points. Employee #1 fell 50 feet 11 inches to the deck and was pronounced dead at the hospital.
