Investigation Summary
At approximately 2:30 p.m. on October 16, 2001, Employee #1 was conducting a maintenance check of the heaters on the roof of a single level office building in Denver, Colorado. The distance to the top of the roof was approximately 18 feet. While standing on the metal ladder, which was placed against the side of the building, Employee # 1 fell approximately 18 feet to the ground. He died from the impact. The ladder used to gain access to the roof was unsecured and did not reach the top of the roof.
