Employee killed from fall off of ladder

On June 13, 2001, Employee #1 was laying roofing paper on a pitched roof when it began to rain. He walked towards an unsecured ladder on the west side of the building to exit from the roof. The ladder was an aluminum ladder with the feet sawed off and was wet from the rain. As Employee #1 accessed the ladder, the base of the ladder slipped out causing him to fall approximately 18 feet to the ground. The impact of the fall caused severe trauma to the head. He was transported to the hospital where he remained on life-support for 2 days before dying of his injuries.
