Investigation Summary
On February 8, 2001, Employee #1, a welder and an independent subcontractor, died from a concussion when he fell off a ladder while working on the Number 2 Mud Pump. The pump overpressure (pop-off) valve tripped open. Employee #1 was working on a ladder directly in front of the overpressure valve and was struck by the vented mud stream. He was knocked off of his ladder and struck the ground where he was found by other workers at the site. Employee #1 died a short time later from apparent head injuries and concussion.
