Investigation Summary
Employee #1 got his forklift stuck in the mud. A coworker attempted to use a second forklift to lift and remove the stuck forklift. Both Employee #1 and the coworker were still in their respective operators seats. Employee #1 was struck in the head and instantly killed by the forks of the second forklift when its forks became dislodged from the first forklift during the lift.
