Investigation Summary
On July 5, 2000, Employee #1, Forklift being driven with forks carrying a load of pallets traveling from west end to east end of warehouse. Forks were raised 61 x 3/4 in. off the floor so operator could not see. Corner of pallets struck a pole causing the forklift to overturn. Driver was not wearing a seat belt and was either thrown off of forklift or climbed off and was crushed by the rops. He sustained fatal head injuries.
