Employee killed in fall off roof

On April 13, 2000, Employee #1 fell off of a roof while putting shingles on the roof. According to the other two employees who were on the site, there were no actual witnesses to the accident. They both stated that when they last saw Employee #1 he was wearing a harness and they don't know what happened. One employee was off of the roof and the other was on the other side of the peak. The employee who was off of the roof stated that Employee #1 had stated that he would be right down, and at that time he was wearing his fall protection. The builder that this employer was working for stated that he kept on them and that the men were always tied off when he observed them. This employer had a written policy regarding fall protection and it was signed by Employee #1. The builder of the house where the accident occurred stated to the inspector that whenever he passed the house the employees had been wearing fall protection, and had always worn it during the years the employer had worked for him. Employee #1 fell off the roof and suffered fatal injuries from the fall.
