Employee killed when knocked down and crushed by pipe

Employee #1 attempted to assist a forklift operator in off loading four stainless steel pipes, measuring 3.25-in. in diameter and 35.188 ft in length, on to a 40 ft flat bed trailer measuring 57 inches high and 8 ft wide. Employee #1 was last observed standing on the southwest corner of the trailer when he was struck above his left knee by one of two rolling stainless steel pipes. The second pipe followed the 1st pipe and Employee #1 to the ground. He was found dead with a massive head injury.
