Employee killed when crushed between hoist bucket and beam
Employee #1 was caught between a skip hoist bucket and a steel beam when he attempted to enter the area to lower a chain guard while the hoist was being raised. Employee #1 suffered severe head trauma, and was pronounced dead at the site.
