Investigation Summary
On November 18, 2000, Employee #1 was ascending a ladder inclined upon a building, when he fell. He was attempting to reset the electrical switch of the kitchen exhaust fan that was located on the roof. An individual found him lying unconscious on the ground with the ladder next to him. Emergency medical services were summoned and he was transported to the hospital. At the hospital, he received treatment for multiple injuries, including a right femur fracture, orbital bilateral fracture, right frontal brain, and gallbladder trauma.
