Investigation Summary

Investigation Nr: 171060528
Event: 01/31/2001
Employee Suffers Amputation Of Fingers And Thumb

Employee #1 was the acting supervisor at the time of the accident. Employee #1 noticed that a coworker operating the saw was pulling the blade through the material (1-in. acrylic decorative rod) too slowly. This was creating acrylic slag buildup around the material. Employee #1 went over to correct the coworker, reached down with his left hand and pulled on the slag to remove it from the trough (cutting jig). When it would not come loose, Employee #1 pulled harder and in the direction of the saw blade. The saw blade was approximately 12 in. to 15 in. away from the cutting jig and the rotation of the saw blade was forward/down. The slag came loose and the momentum put Employee #1's hand into the saw blade, cutting off all his fingers and thumb. Employee #1 was hospitalized.
