Employee crushed by car and truck

An employee was working underneath the front end of a 1997 Oldsmobile. The car was suspended in mid air. He was removing bolts from an engine bracket that happened to be the only piece connected to the automobile from the hook of a boom truck. The employee was in the process of obtaining a cylinder head from the engine. The Ford boom truck was used to hoist the front end of the automobile by connecting a hook to the engine bracket. Two safety chains, according to the company's procedures, are to be used whenever hoisting a car. This had not been done. When the bracket was unbolted, the car fell, crushing the employee between the boom truck and automobile. He died from his injuries.
