Investigation Summary
Employee #1 was cleaning truck trailers in the cold storage cooler warehouse. He was planning to start on a semi-trailer in overhead bay door #74, but was having a hard time getting the dock plate to work. He pressed a button on the dock panel to it, but the plate would not go down. He then removed the locking pin and the "Do Mot Remove " tag on the side of the plate. The pin had been placed through the dock frame to lock out the system and to keep it in an upright position. Employee #1 then released a second safety device, a latch to keep the door from coming down prematurely. This latch is normally released by pulling a chain extended through the dock plate, allowing the hydraulics to slowly lower the plate to the level of the trailer. After the second device was released, the plate still would not go down. Employee #1 then stood behind the plate and began pulling down on it. The plate dropped, knocking him to the floor and pinning him underneath it from the waist down. He yelled for help and coworkers in the area rescued him from under the plate. Employee #1 was treated by EMTs and then transported to the University of Utah Medical Center, where he was treated and released.
