Investigation Summary
Employee #1 was cleaning a cookie dough continuous mixing machine by hand using a large plastic scraper. He attempted to scrape the rotating mixer blades. His hand got caught in the blades as they rotated past the end of the opening. The blades pinned Employee #1's left hand in the mixer, amputating it. Employee #1 was hospitalized for his injury.
