Employee injured by fall down elevator hoistway

On October 30, 2000, Employee #1 was setting a ladder into an elevator hoistway that measured 88" wide and 69" deep. The height of the hoistway from the 2nd floor level measured 12'10. Employee #1 lost control of the ladder and fell into the hoistway, down to the pit a distance of 16' where Employee #1 landed on his feet and then fell onto his back. The ladder was extended to a length of 13'5". The maximum length possible from the opposite immediate corner of the hoistway to the leading edge of the floor would be 14'3/4". The hoistway was protected by a full guardrail system. Employee #1 was hospitalized.
