Investigation Summary
Employee #1, an inspector at a pipe casting plant, was operating a pressure testing station on pipes of various sizes. A hydraulic hose ruptured next to the operator's station, atomizing fluid at 1,200 psi and spraying it directly at a gas-fired floor heater next to Employee #1. The cloud of hydraulic fluid ignited, and a large fireball engulfed an area of approximately 20 square feet. Employee #1 sustained severe burns on his back, ears, arms, and hands.
