Investigation Summary
Employee #1, age 18, was helping to transfer metal. He had placed his foot in the table uplift of a conveyor system when the head stock operator activated the machine, crushing Employee #1's foot between a floor beam and a support beam. He was treated by EMTs and then transported to the hospital, where foot was surgically amputated. Employee #1 violated lockout/tagout procedures.
