Investigation Summary
Employee #1 was a ground worker on a tree trimming operation. Employee #2 was in a tree and cut loose a branch. Employee #1 let go of the guide/safety rope, stepped to the side, and then forward under the falling limb, which struck him on the head. He was killed instantly. Employee #2 suffered from post-accident stress and has not worked since the accident. He apparently left the state and his employer does not know his location.
