Employee's skull fractured in fall from ladder

At approximately 3:05 p.m. on July 31, 2000, Employee #1 placed an extension ladder against a 16 ft high pipe and climbed it in order to unscrew a valve on the pipe. As he undid the valve, the pipe swung free and knocked Employee #1 to the concrete floor. He sustained a fractured skull and multiple fractures of his arms and legs.
