Investigation Summary
On October 12, 2006, Employee #1 and a friend went on a hike in rugged terrain adjacent to the cement plant. During the hike, Employee #1 stopped several times and had a distress appearance. He was last seen entering his office by a coworker. She heard a groan and then a thump. After entering the office, she discovered him on the floor of his office unconscious. She immediately notified plant emergency response personnel as well as placing a call to emergency medical personnel. The plant responder arrived and administered cardiopulmonary resuscitation along with a defibrillator but could not revive him. The American Medical Response Unit responded to the emergency medical personnel call. They arrived and began providing medical care, but failed to revive him. He was transported to a medical center and was pronounced dead on arrival. The cause of death was a myocardial infarction.
