Employee Struck In The Chin By Flying Object

On November 25, 2003, Employee #1, a miner, was changing v-belts on an aggregate plant motor. An all threaded bolt that was welded to a job made eye broke when it was placed under tension from a come-a-long. The all thread bolt assembly flew upwards striking Employee #1 in the chin. Employee #1 was hospitalized with a fracture.
