Investigation Summary
At approximately 10:30 a.m. on November 30, 2001, Employee #1 was operating a part- revolution power press in a fabrication department. The press stopped when the light curtain was broken by another operator. The control operator used a key to put the press in an inch mode to bring the ram back to the top. When this was done, Employee #1 rushed in to retrieve the part when the ram came down on her hands. Employee #1 lost several fingers and crushed both hands and was hospitalized.
