Investigation Summary

Investigation Nr: 200372019
Event: 12/27/2001
Employee killed when struck by steel bridging bundle

On December 27, 2001, Employee #1, an owner operator truck driver, was delivering a load of structural steel weighing approximately 15,000 pounds to a job site under construction. Employee #1 backed the load on his first attempt and then decided to pull forward to the unloading area. Upon trying to unload the materials, Employee #1 unhooked the first strap near the front of the trailer and proceeded to remove four additional straps down the length of the trailer. When Employee #1 got to the last strap and loosened it up, a 20-foot-long bundle of steel bridging slipped from the trailer and struck him in the head. Emergency Medical Services was summoned and Employee #1 was taken to the hospital where he died five days later.
