Investigation Summary
Employee #1 and a coworker logging. Employee #1 was operating a Hydro-Ax clearing select trees from a loading deck area used to load the logs onto trucks. The coworker was operating a Timberjack 460 Skidder clearing the deck area. Employee #1 got off the Hydro-Ax and went to discuss the job with the coworker who also climbed down off the skidder. After the discussion, the coworker got back on the skidder, put the skidder into reverse, and backed up. Employee #1 was crushed by the right rear tire of the skidder and killed. The reverse alarm was operable.
