Forklift Operator Killed in Forklift Tipover

On February 20, 2001, Employee #1, a forklift operator, was crushed by a forklift. Coworkers were placing bundles of insulation on the roof of a building under construction with a Lull type forklift being operated by Employee #1. The ground was uneven and the bundles being lifted from the forklift caused it turn over and crush Employee #1. Employee #1 was killed.
