Employee injured in fall while descending ladder

Employee #1 was climbing down a 50 ft tall ladder of pylon driving leads when he fell approximately 20 to 25 ft to the ground. He sustained injuries that required hospitalization. The ladder's rungs were bent, broken, or missing, and the siderails were also bent. Employee #1 was not using any form of fall protection or arrest while climbing or descending the ladder.
