Investigation Summary
On January 30, 2001, Employee #1 fell from the rear of a Crown platform lift while in the process of relocating two pieces of furniture, a corner wedge of a three piece sectional and a recliner. Employee #1 was provided with a tether and retractable life line and was at the 7th level in aisle 28/29 when he fell 27 ft. Employee #1 was killed.
