Investigation Summary
Employee #1 was unloading a trailer when he apparently stood up and looked through the forklift's telescoping mast to read a label. He accidentally engaged the mast controls and the mast lowered, catching his arm, neck, and head between it and the base of the forklift. Employee #1 was killed.
