Employee Is Killed in Fall Onto Concrete Floor

On June 4, 2000, an industrial truck operator was disassembling copper parts in a junction box when he came into contact with a 480 volt three phase bus. After being shocked, Employee #1 fell approximately 51 in. onto a concrete floor and was pronounced dead at the scene. The employee was not an electrician, and he was not authorized to perform this work.
