Employee killed after falling from ladder

On November 10, 2001, Employee #1, a machinist, was performing repair work on a printing press, working from a movable ladder which was attached (by an overhead rail) to the machine. Employee #1 was positioned on the ladder approximately 6 ft from the ground. The ladder was 1 to 2 ft to the side of the opening in the machinery through which the work was being done. Employee #1 was removing a stuck bolt on a cover of the press in order to replace a broken shaft. When the bolt broke loose, Employee #1 struck his arm against a part of the machine. Witnesses said that Employee #1's arm hit the press with great force. Employee #1 grabbed for his arm and fell from the ladder sustaining head and chest injuries. Employee #1 was killed.
