Investigation Summary
On April 12, 2002, Employee #1 was mixing pizza sauce in a black kettle outside of the building when he was found with a massive head injury, exposing cranial fragments and matter. He had used an electric hand mixer to blend the contents of the kettle. The mixer comprised of a small electric motor resembling a hand drill that was connected to an approximate 3-ft long rotating shaft. The shaft was attached to the chuck end of the drill motor. At the other end, there were three mixing blades surrounded with a guard. The mixer received power through an approximate 20-ft-long orange extension cord, which was plugged into an outside outlet. After finding Employee #1 with the head injury, emergency medical personnel was summoned. They arrived and transported him to the hospital. At the hospital, he died from the traumatic injuries sustained to his brain. An autopsy was performed and the results indicated that a gunshot had caused the traumatic head injury.
