Investigation Summary
Employee #1 was working on a flatbed trailer attaching a load of three steel beams, weighing approximately 3,000-to-3,800 pounds with slings to a Lorain MC 790 Friction Crane. The load was raised, and Employee #1 was working in close proximity to the suspended load. As the load was lifted approximately 10 ft, the hoist lever disengaged, causing the load to fall on top of Employee #1. Employee #1 was killed.
