Employee killed when struck by drill rig counterweight

A drilling supervisor was taking measurements from a recently dug hole. He was bending over the outer casing surrounding the hole while the drill rig operator rotated the main attachment 180 degrees to drop the excavated soil into a temporary retention pan. The operator was unaware that the supervisor was viewing the hole. As the operator swung the drill around to make the drop, the rear counterweight struck the supervisor in the head and killed him.
