Event: 03/21/2000
Employee Died From Asphyxiation

A forklift operator was moving pallets of obsolete computers between buildings. The employee had returned to his regular building with a pallet load of computers and entered onto the incline ramp. The employee got off of the forklift and went to a door on the west side of the building in order to unlock the door he needed to enter. A coworker saw him go by and open the double door. After a few minutes, the coworker noticed the door was still open. The coworker went to close the door and saw the forklift at the bottom of the ramp with the pallet of computers pinned against the building wall. As the coworker approached the scene, he saw the driver pinned between the forklift and the palletized load of computers. The driver was extracted and taken to Tampa General Hospital where he was pronounced dead. Hillsborough County Medical Examiner stated, death was from asphyxiation due to severe mechanical compression of the chest. Micro fractures to the ribs were found but there were no full break(s) in the bones or damage to internal organs due to the accident.
