Two employees injured and one killed by falling scaffolding

Employees #1, #2, and #3, all from Precon Corporation, were working inside of a 20-million-gallon concrete water tank. They were dismantling the support scaffolding used to shore up the dome top when it was being poured. The employees began to move one section of the scaffold, however, the safety line attached between the top of the 50-ft-high scaffold was not disconnected. As the scaffold was moved away from the adjacent scaffold, the safety line caused the adjacent scaffold to teeter, sway, and then to fall. In so doing, the safety line snapped and broke, which caused the scaffold to fall in the direction of the employees who were running to escape it. Employee #1 received minor injuries which were treated on site. Employee #2 received a broken leg and was hospitalized. Employee #3 received a broken leg and head injuries, and was hospitalized. Employee #3 died from a concussion.
