Investigation Summary
At approximately 3:00 p.m. on May 9, 2001, Employee #1 finished his clean up duties and was sitting on an idling forklift waiting for the shift to end. Employee #1 was playing with the controls of the forklift moving the forklift back and forth. Employee #1 was told several times by a coworker to stop. The coworker later heard the forklift running and Employee #1 yell as the forklift backed off the dock. The forklift flipped over and landed on the employee, crushing him to death.
