Employee Killed Falling From A Ladder

Employee #1 died when he fell thirty feet from a ladder. He was climbing a ladder to access the roof on the third level of a building. The ladder was unstable. The ladder kicked out, and Employee #1 fell thirty feet. Employee #1 did not have prior ladder training.
