Employee killed from fall of 18 feet

Employee #1 was installing insulation board on the front outside structure of the house. He was standing on the rafters and on the top plate of the front outside wall on the second story of the house. The other employees were getting ready to break for lunch. Employee #1 pulled himself in and pulled out the stud he was holding and fell to the ground. He fell 18 ft and died.
