Investigation Summary

Investigation Nr: 201281011
Event: 11/14/2000
Employee Killed by A Falling Wall

On November 14, 2000, Employee #1 died when a lift precast slab concrete wall fell on him. The lift slab was approximately twenty-five feet high. The truck crane was positioned inside the building at approximately thirteen feet from the wall with outriggers set. The boom was fully extended sixty-three feet to lift over the wall. Eight columns had been rigged for the lift from a trailer positioned on the outside of the building, approximately fifteen feet from the wall. During the lift, the load became unstable causing the truck crane to topple into the wall. Employee #1 leaped from the controls of the crane to warn fellow workers of the possible wall collapse and was killed by the falling wall.
