Employee falls from a truss and dies

Employee #1 was helping to install wooden roof trusses, 40 ft high on a four story apartment building. He was releasing a load of multiple trusses from a crane when a single truss underneath the stack shot out. This truss struck him, knocking him backward about 5 ft, and he fell 41.3 ft to a concrete surface and died.
