Employee Falls 32 Feet Not Using Fall Protection

Employee #1, a carpenter working for Bob Anderson Builders, Incorporated, suffered from multiple injuries when he fell 32 feet from a building while not using any fall protection. He was walking on a 6:12 pitch plywood roof. He slipped on the frost and slid off the roof landing approximately 32 feet on to the ground. Employee #1 was sent on to the roof to find and work with another employee who was working on the north east side of the building. The access to the roof was located on the far west side of the building. Apparently, Employee #1 was not visible from this location. Several reasons caused Employee #1 to fall. The first reason was the location of the roof access. Secondly, there was a lack of communication to the employee. Next, Employee #1 did not use fall protection. Finally, there was frost on the plywood. Employee #1 had only worked seven days for his employer. His employer had used slide guards on parts of this building and also guardrails. However, where the employee fell, there was a slide guard which was approximately 49.5 inches away from the valley where the employee fell. The slide guards on this roof were not a means of fall protection. His employer was warned previously to use fall protection. He went to a local hospital for treatment.
