Investigation Summary
Employee #1 was unloading a trailer using a Clark forklift. He left the operator's position and got in between the forklift and the trailer. The brakes on the forklift were not set. The forklift rolled forward, crushing his midsection area. Employee #1 sustained abdominal injuries and was hospitalized.
