Employee killed when struck in head by falling debris

Employee #1 and a coworker where working in a shallow trench directly below a crew removing a falsework. The crew was working 18 levels, nearly 200 ft, above Employee #1. Employee #1 was killed when a 4-in. by 6-in. wood member from above struck him in the head. The crew removing the formwork sent a laborer down from the roof level to the ground to serve as a spotter to warn people away from the area where debris fallout might be reasonably anticipated, however this spotter did not speak English and Employee #1 may not have understood the need to leave the area where he was working. No physical barricading devices were utilized at ground level. Guardrails without toe boards had been erected at the main roof area, and a vertical debris net had been hung at the main roof, but the area of collapsing forms and shoring unexpectedly extended beyond the area enclosed by the debris netting.
