Employee dies from fall through open elevator shaft

On February 20, 2001, Employee #1, a laborer, had been assisting coworkers with replacing broken sprinkler pipes on the 2nd floor of a building. Employee #1 left coworkers to go find a cheater bar and was later found in the elevator pit. It appeared that Employee #1 had fallen through an open elevator shaft. Employee #1 was killed.
