Employee Killed in Fall From Elevation

Employee #1 was using a yard truck to unload a roll on-roll off vessel. Near the end of the procedure, he pulled his yard truck into the vessel and got out to wait for his turn to pick up a trailer. The trailer was to be brought up on the elevator from the well deck. The elevator started down to get the trailer. He returned to his yard truck and began to back his vehicle up to the guardrail in front of the elevator. He misjudged the distance and backed through the barricade. The vehicle fell 30 feet into the opening, killing him
