Investigation Summary
Employee #1 was sitting in a maintenance golf cart while Employee #2 was spreading fire ant poison in a section of a development. The golf cart was parked along the side of the road that runs the circumference of the development. Employee #2 had just sat down in the cart and was waiting for Employee #1 to record the location of the fire ant eradication when the cart was hit from behind by an automobile driven by a resident. Both employees were thrown from the cart with Employee #2 landing in a grassy area and Employee #1 was thrown into a tree. Employee #1 was killed. Employee #2 was hospitalized.
