Fork Lift Runs Over Employee

Employee #1 died when the fork lift ran over him. The fork lift had "no visible model or serial number". Apparently, there were no witnesses, and no one knows what Employee #1 was doing at the time of the incident. He was instructed to check the oil level of a "Hyster Number 4" fork lift truck in the yard. No one knew there was an accident until the fork lift backed into an upright which supported a work shed. The fork lift ran over Employee #1's torso and kept on reversing until it hit the upright.
